If you believe or have evidence of SNAP benefits fraud, it is important to report it right away. Fill out the online form at the USDAs Office of Inspector General website. Your complaint can remain confidential you can include your name or opt to report the fraud anonymously. Another option is to call 202-690-2474 to make the complaint by phone. You can also mail a written letter detailing the suspected fraud to USDA, OIG Hotline, P. O. Box 23399, Washington, D.C. 20026-3399.

Requesting A Replacement Card
Since Alabama uses the EBT card for Medicaid as well as food and cash assistance, you must call the appropriate number to cancel your lost card. For Medicaid, the toll-free number is 1-877-391-4757. For all other benefits, . This automated system is available 24 hours a day, seven days a week. First, call 1-888-328-6399.

Can I use my card right away? No, you will need to create a Personal Identification Number by calling the EBT customer service toll free number in order to activate your card. Once the PIN is created, it will be the same for all replacement cards unless you call the EBT customer service toll free number to change the PIN.
What is a Personal Identification Number? A Personal Identification Number is a four-digit secret code that allows only you to use your Hoosier Works Card. Never tell your PIN to anyone! If someone knows your PIN, he or she can use your card to get your benefits and those benefits will not be replaced. It is best to choose four numbers that are easy for you to remember, but hard for someone else to figure out. Do not write the PIN on the card. Receiving A Replacement Card
After you call the appropriate number, Alabama DHS mails your replacement card to the address on file. If you’ve recently moved or the address in the system is incorrect, you must call your case manager as soon as possible and have the address corrected before your card is mailed so it will go to the right place.
Expect to receive your replacement card within five to seven business days. You can call the same customer service line you used to make the request to check on the status of your request. If you get cash or SNAP benefits and you lose your EBT card, DTA may charge $5.00 for replacement of the EBT card depending on the situation. DTA will deduct the $5.00 from your cash or SNAP benefits in your EBT account .
DTA should not charge the $5 fee for a replacement EBT card if:
- the EBT card was lost in the mail and you never used the EBT card that was sent to you,
- the EBT card did not work correctly, ,
- you are a victim of domestic violence,

Requesting A New Card
As noted above, you can report your card lost or stolen by phone or online in most states. If you report the incident online, you may still need to call your states EBT customer service phone number to request a replacement card most online systems only allow you to report the cards status and wont automatically send you a new card.
When you call the customer service department, a representative or automated system will verify your identity using your current EBT card number or other personal information, such as your date of birth, EBT PIN, and/or Social Security number.
